2013 Insurance Benefits Guide

General Information
All full-time employees have the option to enroll in benefits. A new full-time employee will attend a payroll/benefits session that will provide details regarding specific benefits available.
For DCSD Employees already enrolled in Benefits
There are special eligibility situations that allow employees already enrolled in Benefits to make changes, however, the employees must notify the Benefits office and make the changes within 31 days of the event.
For example, if an employee's spouse loses health coverage and the employee wants to add him/her to coverage, then a "Notice of Election" form must be completed within 31 days of the loss of coverage. If the employee fails to complete the form and submit proper documentation required within the 31 days, he/she will have to wait until the next enrollment period to add the spouse to coverage. This would also be true for children.
Enrollment periods are the month of October every year.
During ANNUAL ENROLLMENT, eligible employees, retirees, survivors and COBRA subscribers may change health plans. This includes changing to the Medicare Supplemental Plan, if you are RETIRED.
During OPEN ENROLLMENT, which occurs in odd-numbered years, eligible subscribers may enroll in or drop their own health coverage and add or drop their eligible spouse and/or children.
ORP (Optional Retirement Plan) Annual Enrollment Period is January 1 thru March 1 for all members. During this time ORP members may choose to:
- Change investment providers vendors
- Switch to SCRS if member has between one(1) and five (5) years of State ORP service.
403(b) Plan Universal Availability Notice for 2011 (No changes made for 2012)- DCSD is a sponsor of a 403(b) program established and maintained under regulations issued by the IRS. As an employee eligible to participate in the Plan, you are permitted to defer a portion of your wages into an investment account approved for the Plan.To view more details click on the following link: 403B Annuity Availability Notification.
If you — or anyone you cover under your health plan — smokes or uses tobacco products, you are obligated to pay a surcharge of either $40 or $60 per month. The use of tobacco applies to all insured employees and insured dependents 18 years of age or older.
